It is mandatory for children who are aged 6 years of age or older to obtain a School ID Card.
Below you will find useful information and helpful links provided to help you obtain a School ID Card.
Polish School ID Card

About the card.
It is available to students from the age of 6 and is compulsory in our school!
The ID card does not contain a photo of the student, so it is only valid with another document confirming identity.
It entitles you to purchase a discounted ticket in public transport and cultural institutions during your stay in Poland.
It may be useful when obtaining a passport for your child in Poland (discounted fee).

When to apply?
At the beginning of the school year (in September/October), we collect completed forms from all students (including ID cards requiring extension of validity) and send them to the Polish consulate in Manchester. After receiving the ready ID cards, we give them to parents.
If someone would like to obtain/extend the ID card beyond the above deadline, they should complete the form, contact the admin office to confirm the child’s attendance, and then send it to the Consulate on their own to the following address:
Consulate General of the Republic of Poland in Manchester (school id)
Manchester One Building, 51 Portland Street, M1 3LD Manchester
How to obtain/Extend the card?
You can ask for a school ID application form at school or by clicking the button below.
It must be completed and signed by hand (the signature cannot be scanned and printed, as the form will be rejected by the consulate).
